Photo Release

I hereby give permission for my child to be photographed during Camp Prep 2024. I understand the photos will be used to advertise activities, to share during marketing and/or reports and for promotional purposes including flyers, brochures and on the internet. I understand that although my child’s photograph may be used for advertising, his or her identity will not be disclosed, I do not expect compensation and that all photos are the property of Charlotte Preparatory School and Camp Prep 2024.

Field Trip Release

I give permission for my child to attend all scheduled field trips. I am aware that Camp Prep will communicate with me 1-2 weeks in advance of the scheduled field trip. If I do not want my child to attend a specific field trip, I will provide an email to the Camp Director.

Camp Prep 2024 are not responsible for lost or damaged personal property. We will keep lost property in a specific area until the end of our summer camp. All scheduled events/field trips are subject to change.

I understand that refunds are available two weeks or more prior to the start date of camp. A refund fee of $150 will apply per camper.

Children's photos and quotes may be used for publicity purposes.

In case of an emergency, and if a family physician cannot be reached, I hereby authorize my child to be treated by Certified Emergency Personnel (i.e. EMT, First Responder, and/or Physician).

Participation Consent

I voluntarily agree, on my own behalf and on the behalf of my child/camper, to release and discharge Charlotte Preparatory School, Camp Prep 2024 and its employees from, without limitation, any and all actions, causes of action, claims, demands, damages, costs, - OR - expenses, compensation, and/or suits at law or in equity, arising out of or relating to any act or omission by Charlotte Preparatory School, Camp Prep 2024 and its employees in connection with my child attending Camp Prep 2024 AND/OR being driven on the Camp Prep bus.

Summer Camp Tuition and Payment Policy:
Summer camp tuition fee is due at the time of online registration, by credit/debit card or FACTS account. If a camper’s summer camp tuition is not paid they will not be able to attend camp.
